Form of Consent for Personal Data Collection, Processing and Use
According to the Article 8, Paragraph 1 of the Personal Data Protection Act, ABEZ Creative INC. (hereinafter referred to as the “Company”) issues its statement of personal data protection and collection agreement to notify you of your responsibilities and rights and solicit your consent to the collection, processing and use of your personal data by the Company. Your signature below indicates that you have read, understood and accepted the contents set forth in this agreement.
- Purpose of Collection: The personal data is collected for the purpose of contacting and conducting cooperation negotiations related to the East Coast Tribal Working Holiday program.
- Categories of Personal Data Collected: The personal data to be collected includes your name, organization or employer name, National Identification Number, contact telephone number, household registration address, National Identification Card information, email address, etc.
- Time Period, Territory, Recipients, and Methods of Use of Personal Data:
•Period: The Company will retain and use your personal data for the period necessary to fulfill the above-stated purpose and as required by applicable laws or regulations.
•Territory: Your personal data will be used within the territory of the Republic of China (Taiwan) and in other jurisdictions where the transfer or use of personal data is not restricted by the relevant central competent authority.
• Recipients: Your personal data may be provided or disclosed to the following parties:
• The Company and affiliated entities: The Company itself, the Company’s head office, and the Company’s affiliated enterprises (including those of its head office).
• Service partners: The Company’s correspondent bank and any third-party institutions or service providers contracted by the Company to assist in its business operations.
• Domestic and international authorities or recipients: Recipients of personal data via international transmission (where such transfer is not prohibited by the central competent authority), as well as government agencies or financial supervisory authorities that are legally entitled to request or obtain such data.
• Co-marketing/cooperative partner: The Company’s joint marketing or cooperation partner, the East Coast National Scenic Area Administration.
• Method of Use: Personal data will be collected, processed, and used by means of automated systems or by other non-automated measures in compliance with the relevant laws and regulations.
- Rights of the Data Subject: You may exercise the following rights with regard to your personal data, as provided under Article 3 of the Personal Data Protection Act:
- Access and Copy: You have the right to inquire about or request to review your personal data held by the Company, and to request a copy of your personal data. Please note: the Company may charge a necessary fee, in accordance with the law, to cover the cost of providing any copies of your data.
- Supplement or Correction: You have the right to request that the Company supplement or correct any of your personal data. Please note: under relevant laws, you may be required to provide a proper explanation or proof when requesting correction or amendment of your data.
- Stop Collection/Processing/Use and Deletion: You have the right to request that the Company cease the collection, processing, or use of your personal data, and to request deletion of your personal data. Please note: if your personal data is necessary for the Company to perform its legal duties or business operations, the Company may refuse to comply with such a request in accordance with the law.
- Consequences of Not Providing Personal Data: Provision of your personal data is voluntary. However, if you choose not to provide the relevant personal data, the Company will be unable to conduct the necessary review and processing of your application, which means it will be unable to complete the registration or related procedural requirements for the East Coast Tribal Working Holiday program. This may affect your rights or interests in participating in the program.
